Welcome to the DHS online Application for Support Enforcement Services. A service fee will be applied to filings with payment. Use this application if you are applying for child support enforcement services, which include the following:
- Searching for the other parent
- Legally establishing paternity
- Establishing a court order for child support and health insurance coverage
- Collecting support payments
If you are the custodial parent, complete a separate application for each noncustodial parent from whom you want support. Please note that in order to use this online application, you must have a social security number and the number of children on the application cannot exceed six. If you do not have a social security number, or if you have more than six children, please download and complete this paper application. About the Application
The application consists of the following sections, all of which must be completed:
- Applicant (details of the relationship between the applicant and the children)
- Indicating family violence will impact the quality of service provided by restricting the amount of information that can be shared with and obtained from the federal government and other secure resources.
- Custodial Party (name, address, contact and personal information, employment information, details regarding the nearest relative)
- Noncustodial Party (name, address, contact and personal information, identification, parental and employment information, details regarding the nearest relative, and child support) Additional information for custodial or noncustodial parties, including a question on family violence, is also requested. For this question, you may provide corroborating evidence (e.g. police reports) or reason to believe that the disclosure of such data might result in physical or emotional harm to a custodial parent, noncustodial parent or a child in a case. Indicating family violence will impact the quality of service provided by restricting the amount of information that can be shared with and obtained from the federal government and other secure resources.)
- Support (relationship between the parents, and details for each child for whom support is requested)
- Health Insurance (details of any health insurance coverage for the children)
- File Upload (is not required, but will allow for expedited processing of your application. You may upload documents — corroborating evidence for family violence; photos; Court Orders regarding child support, divorce proceedings or paternity, etc. — to support your application.)
- Signature (prior to submitting to DHS, the application must be signed electronically)
